Women and Testosterone Replacement

Yes, you read that right. I wrote Women and Testosterone Replacement.

Hormone Replacement and Bioidentical Hormone Replacement with Estrogen and Progesterone creams and pills have received a lot of interest for more than 60 years. Any Regenerative Medicine doctor who is evaluating your hormones should include Free and Total Testosterone in the evaluation of any woman considering hormone replacement therapy. The balance of the sex hormones is equally important to the amounts that are safely being replaced. Science of NAD Therapy in Thailand and it’s Effects on Mitochondria
I haven’t been this impressed by an emerging Regenerative Therapy in the last 15 years.
Before ever recommending a new therapy to a patient I always do as much research as possible and try the therapy myself. A concerned and dedicated doctor would never recommend a therapy in Regenerative Medicine that he would not give to himself or his family. I’ve done both with NAD.

Aging and Mitochondria

 

 

As we get older and as our hormone levels drop, mitochondria also change like our body does. Their shape and their function changes, worsens and declines. A very big reason that they age and fail is the lack of healthy NAD+ to feed the mitochondria to keep it healthy, strong, functional and normal in appearance. And as we get older we stop producing as much NAD+.
 
The research from Harvard University professor David Sinclair simply said the following: “Low levels of NAD are associated with increased risk of heart disease, Type 2 diabetes, Alzheimer’s disease, muscle loss and accelerated aging. NAD levels naturally decline with age. … One, we start producing less NAD as we get older.”
 
Very clearly: once we start producing LESS NAD+ we get older. Less healthy MITOCHONDRIA. Less energy. Less new cell growth. More cell death. Older sicker organs and muscles. Lower immune function. More risk for Cancer.
 
Also, for those patients worried about risks of Dementia, Alzheimer’s Disease and Traumatic Brain Injury there is a lot of evidence that NAD has been effective in slowing or improving patient’s conditions.
 
What exactly is NAD and what is it produced from?

NAD is produced in a process extracting it from Vitamin B3 (Nicotinic Acid). Vitamin B3 is a key tool the body uses to cut fat and promote healthy liver and heart function. NAD is concentrated into a formula that can be given by intravenous infusion. It is safe and well tolerated. 

The IV infusion takes between 1-3 hours depending upon the dose. It is done in the office and patients can go on about their day afterward.

Supplemental Vitamins: The Importance of Vitamin C and D

Vitamin sales worldwide were over 48.5 Billion US dollars just a few years ago and this is projected to increase twofold by year 2025. This is likely due to a few reasons including increased marketing of vitamin products around the world and a greater interest in Anti-Aging, Regenerative Medicine and personal health and wellness.
 

Vitamins have previously been used as a “one size fits all” type of product so there is little detail on what specific reasons specific vitamins should be used for specific diseases or health issues.

I will try to explain smart, targeted vitamin use in this blog based on current science, biology and physiology. We should also keep in mind that too much of anything (even vitamins) can cause a potentially bad response.

There is too much information on vitamins to include in a single article so let’s discuss the  importance of  Vitamin C and D:

Importance of Vitamin C

The most popular vitamin consumed in the world is likely Vitamin C – also known as Ascorbic Acid.

We know that the main natural source is fruits and vegetables. It is important for building collagen which is part of all connective tissues in the body – skin, muscle, joints, and cartilage. We frequently read that Vitamin C is also important in promoting a strong immune system, but we rarely hear specifically why. Here is the theory.

There are 2 important parts to the immune system:
  • Innate Immunity existing cells in your body that attack any foreign substance, virus or bacteria
  • Adaptive Immunity cells called B cells and T cells that “read” a foreign invader, virus, bacteria or other infection and produce a specific immune response to destroy the invader
Vitamin C is a key element in both of these important immune responses. This would relate to your body’s ability to fight infections. This is the reason why large doses of intravenous Vitamin C were used to try to treat severe COVID-19 infections by doctors in both Asia and North America – with reported anecdotal success.
 
Another important aspect of this type of immunity is the body’s ability to recognize early forming cancer cells and destroy them before they can develop into full cancer disease and tumors. Vitamin C has been touted as anti-Cancer, but let’s see if there’s a scientific basis for this claim. Cancer cells have low levels of an important enzyme called catalase. Catalase is key toward cancer cells DNA reproduction and growth. In theory, this low level of enzyme makes Cancer cells vulnerable to high doses of Vitamin C (there are many clinics around the world using high dose Vitamin C as supplemental and sole cancer therapy with limited definitive data). Another scientific theory is that Vitamin C breaks down to generate Hydrogen Peroxide which can directly kill tissue and DNA of Cancer tumors. To know if this is clinically significant we will need to see good studies and clinical trials in the future.
 

Another phrase we hear often is that Vitamin C is the “stress control” vitamin. 

This has a scientific basis as well. When your body is under stress it releases Stress Hormones that try to protect the body, but these hormones can also increase the tendency for Diabetes, Obesity and Anxiety. High doses of Vitamin C have been shown to limit the over-production of these Stress Hormones which maintains the normal body functions of natural hormone production. These normal functions are proper sugar metabolism, healthy sleep and normal sex hormone production.

Importance of Vitamin D

The recent findings regarding Vitamin D over the last decade have changed the approach to many chronic diseases – Including several cancers. We know that we get Vitamin D from the Sun, but that is only the first part of the process in its development. What actually occurs is that sunlight (morning sunlight) activates a sterol in your skin. This sterol then goes to the liver and is converted to the various Vitamin D forms which are actually hormones, not vitamins!

Most people know Vitamin D as being key to the formation of strong bones, but it is a vital part of many other major body and organ functions. In fact, one Biochemist in California found Vitamin D present in 36 organ tissues of the body.
 
Important Functions of Vitamin D

Vitamin D is found in many of the cells of the innate immune system. Remember, these are the immune cells that are always working to fight infections. These are the first line of defense for your body while it develops specific defenses (antibodies) which kill viruses and bacteria more precisely. Equally important is Vitamin D’s role in controlling autoimmune diseases such as Multiple Sclerosis (MS), Lupus and Rheumatoid Arthritis. This vitamin helps to limit the inflammation caused by an overactive immune system and lessen symptoms and severity of these diseases.

  • Lung Function

Vitamin D deficiencies in humans results in lower air volumes of the lungs. Less volume results in less oxygen to your organs. Vitamin D also regulates the inflammatory reaction of the lungs. When the lungs become inflamed by bacteria and viruses, bronchitis and pneumonia can develop. Low Vitamin D levels have been found to be a major factor in patients with COVID-19 who require ventilators and who do not survive the Coronavirus. People who smoke or have asthma or who live with very polluted air should be especially concerned with keeping high levels of Vitamin D.

  • Heart Disease
One of the early researchers on Vitamin D (Fleck, et al.) found that people who lived further from the equator (and further from regular sunlight) had higher rates of heart disease including hypertension, heart attacks and congestive heart failure (CHF). This developed into prospective studies that showed higher rates of these cardiac diseases in patients with low Vitamin D levels. Though not all studies agree, there is a trend toward showing that patients with higher, normal Vitamin D levels have less cardiac deaths than those with low levels.
 

Summary

In summary, the COVID-19 pandemic has brought out the importance of these two essential vitamins in the health, disease prevention and healing processes of the human body beyond surviving Coronavirus infection. We should take these lessons and see where they apply to the risks in our own lives depending on where we live, what environmental stresses we are exposed to and pertaining to our own concerns about longevity and good health.

Neuroprotection and NAD+

Neuroprotection (Protecting Brain Power) and NAD+ (Nicotinamide Adenine Dinucleotide)

We humans are living longer and need neuroprotection.

We have developed great advances in staying alive longer, looking better, staying stronger and conquering chronic diseases. We have diets, exercise programs, carbohydrate reduction, mega-vitamin therapy, stem cells, peptide therapies, detox and other regenerative treatments.
If we live longer, we must, however, give attention to preserving our brains and cognitive ability. Otherwise these gains in longer life and younger appearance will not mean much. One of the greatest fears I hear from my patients is their concerns about losing memory and mental sharpness as they reach their 40’s and 50’s and beyond.
 
We all have parents, friends or relatives who are losing their mental sharpness and brain power. This may be from a chronic disease like diabetes or heart disease or could be from a disease affecting the brain like stroke or Parkinson’s disease or Senile/Alzheimer’s dementia. This can be quite painful to watch our loved ones start to lose their ability to respond and speak and show emotion. I think we’ve all been through this enough to know how helpless and sad it feels.
 
These are the healthy choices we have to help protect the brain from mental and cognitive decline:
  • Low sugar/low carbohydrate diets
  • Prevention and proper treatment of Diabetes Mellitus
  • Regular exercise
  • Intermittent and regular fasting programs
  • Good control of blood pressure
  • Good control of Cholesterol

  1. Get 7 hours of sleep every night: Sleep deprivation leads to a higher rate of dementia.
  2. Exercise: Regular exercise increases the circulation and vasculature of the brain.
  3. Limit your sugar intake: Alzheimer’s Disease is now called Diabetes of the Brain. It is caused by a lifetime of too much sugar intake.
  4. Try Intermittent Fasting (IF): IF increases MITOCHONDRIAL function in the brain cells. This means more energy, more repair of cells and important neurotransmitters continue to regenerate.
This MITOCHONDRIAL function is becoming the key factor not only in keeping strong brain function, but also in nearly every aspect of Anti-Aging and Regeneration. Very simply, if the cells in your brain and body are being stimulated in the same way as when you were 30 years old, they will also act that way. They will regenerate faster. They will heal faster. They will function with more efficiency. This is why I like the science and effect of NAD.
 
When we add NAD to the bodythe MITOCHONDRIA believe that they are much younger – maybe even 20 years younger. They produce more energy, faster and more focused brain activity as we’d expect in a younger person. The brain can produce important, vital hormones more effectively.

Protect Yourself Against Covid19

Because I’m an Infectious Disease specialist I’ve been getting a lot of messages from friends asking my opinion on what should be done to prevent Coronavirus 2019 nCoV. I may sound a bit cavalier having been personally involved with more serious infections like Ebola, HIV, Dengue and Malaria for 25+ years, but there’s a common thread of advice here:

First, don’t panic. Look at the numbers and then draw conclusions. Thailand has low and slow growing sporadic numbers of cases. Most have already been sent back, so if there’s a small growth afterward there likely wasn’t much further exponential transmission, as in China.

Second, follow the rules – the same rules we had when exposed to potential populations with Ebola or Influenza:

  • Wash your hands. A lot!
  • Stop touching every surface when you are in public, at shops, at mass transit and when it can be avoided.
  • Frequently clean the surfaces of your home and work with a good cleanser
  • Stop touching your face 40 times an hour!!! I know, tough to do because it is so automatic, but know that most infections are spread this way-not through droplet infections passed by sneezing and coughing.
  • If you have children it’s probably best to keep them out of activities with lots of group touching as happens in many school activities. The school should be aware of this. If not, make them aware.
  • Maintain a strong immune system. Get enough sleep, ease up on the alcohol, exercise and take supplements that can help you become a more efficient immune machine (feel free to message me if you have questions). There’s some great immune boosters available in BKK.
  • Know that death due to 2019 nCoV occurs in “at risk” populations: elderly, frail, chronically diseased and immunosuppressed people. Everyone else should recover fully, as we eventually do from the flu.
2019 nCoV will eventually burn out. The virus runs its course when it is not transmitted to a new host. Prevention as above will bring this about faster in the lands outside of China. Unfortunately the Chinese have hit a raw volume of cases that has hit the exponential growth curve and that’s why flights and travels have been restricted in many countries (but not Thailand).
